PT/OT/ST Forms & What To Expect

What To Expect

On the first day, you will need to have a referral from a physician stating what services are needed, a copy of your insurance card, and all completed patient forms. If you are unable to print and complete all the intake forms, please arrive at the clinic 30 minutes in advance to allow time to complete them. Clients will receive comprehensive evaluations in physical, occupational, and/or speech therapy depending on client needs. Evaluations typically last between 45 and 60 minutes, per discipline. During the evaluation, family members and caregivers are encouraged to stay and actively participate in order to answer questions, fill out forms, and assist the client as needed. The evaluation process consists of:

1. Interview and rapport building

2. Standardized and informal assessments

3. Questionnaires and observations

4. Discussion of evaluation/observation

After the evaluation is completed, the evaluating therapist will create a plan of care and measurable therapy goals. The clinician will discuss the evaluation report and plan of care with parents/guardians during the first therapy session. Depending upon client need, each treatment session may vary from 30-60 minutes and may be scheduled for 1-3 sessions per week.

Family members and caregivers are not required to stay during treatment sessions and may leave the facility as long as an emergency contact is on file with the front desk. Please return 10 minutes prior to the end of the session in order to discuss treatment plans and progress with the treating therapist.
